PACE Organization of Rhode Island (Program of all-inclusive care for the elderly) is seeking:

Social Worker:  Full-time position in dynamic health care setting serving frail elders. Must possess a Master’s Degree from an accredited graduate school of social work and have one year experience working with elderly. Bi-lingual Spanish speaking skills preferred. The primary responsibilities of the social worker include completing assessments, assisting in the development of care plans, case management, working collaboratively with the team, and conducting home visits and community liaison activity.
